Discharge Planner Position Summary:

The individual in this position assists in the Case Manager and Social Worker with non-professional tasks. The responsibilities will include, but not limited to the following activities; arrange post-discharge services, fax, make phone calls, communicate with patients, families and other members of the care team.

Qualifications

Discharge Planner Full Time Days candidate will possess the following education, licenses/certifications, and experience:

  • Minimum of 1 year discharge planning experience in acute care or healthcare setting.
  • Working Knowledge of managed care and capitation operations.
  • Organized with excellent verbal and written communication skills and computer literacy.

Pay range: $16.48 - $24.73

Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience
